Secondary hypokalemic non-periodic paralysis in a case of intractable hyperemesis gravidarum – A maternal near miss

Hyperemesis Gravidarum is a debilitating early pregnancy complication causing Hypokalemia and associated morbidity if timely recognition intervention not initiated. A rare sequelae of intractable hyperemesis Non periodic Hypokalemic paralysis. 26 years old primigravida at 13 weeks 6 days gestation was admitted with gravidarum preceded by nausea vomiting 10 duration. Her sonography showed single live fetus corresponding maturity. She treated intravenous fluid therapy parenteral antiemetics. As her continued despite all medications, she started on steroid therapy, however developed hypokalaemic paralysis serum potassium < 2.3mmEq/L ECG changes prolonged QT interval u waves. Timely detection supplementation prevented adverse maternal outcome patient recovered totally within 48 hrs infusion initiation. remains morbid which can be missed in the stages as may remain asymptomatic. It thus becomes essential to serially monitor such patients daily electrolytes especially when corticosteroid for prevent any near miss condition.
